Oasis is a program where independent adults, ages 60+, come for physical, intellectual, and social stimulation. Oasis offers adults the opportunity to be with others for programming, lunch, and/or special activities. The safety of the Oasis participants, JCC members, and staff is first priority.

Listed below are the Oasis criteria for eligibility. Adhering to this set of criteria will ensure Oasis participants have fun in a safe, engaging program. Participants requiring continual “hands on” assistance are better served in adult day care.

• Alert and oriented to person, place and time.
• Ability to recall/retain (most) information.
• Ability to engage in program without cueing(i.e. reminding participant where they are, why they are here, etc.).
• Ability to navigate the physical environment with minimal or no assistance and to move from one program to the next without continual assistance by staff and/or fellow Oasis participants.
• Ability to select from multiple programming choices (i.e. participant is capable of selecting where they want to be each hour of the program).
• Ability to navigate the physical environment, getting around a large space with distance and various program rooms.
• Willing to use adaptive devices (walkers, canes, etc.) when requested by Oasis staff, to make participation in Oasis safer and easier for the participant.
• Falling may indicate lack of physical ability and/or possible cognitive decline and will be monitored by Oasis staff at each occurrence for continued safety and ability to participate.
• Maintain personal cleanliness and proper hygiene.
• Ability to get along with participants and staff with kindness, courtesy, and respect.
• Engages appropriately during programming and requires little to no redirecting during discussion.
• Direct complaints, comments, and concerns to any Oasis staff member so they can try to address them.

Transportation:

• Transportation is ONLY available for Oasis Associates and Levine JCC Members who spend the majority of their time in Oasis.
• Participants must first be approved to ride an Oasis bus. See Oasis staff for details if interested.
• Bus riders must be able to board and de-board the bus with minimal assistance.
• Bus riders must wait for Oasis staff to guide them in boarding and de-boarding the bus.
• Transportation will not be provided to and from assisted living/memory care communities, and those who remain eligible may be required to attend Oasis with a caregiver.
• While there may be wheelchair accessibility on one or more of the LJCC mini buses, the Oasis Staff will determine if it is available for the requested bus route. In addition, bus riders requiring a wheelchair and deemed eligible to participate in Oasis bus transportation may be required to have an aide/attendant accompany them on the bus and during their time in Oasis programming, including while on field trips.
• Seat belts must be used at all times. Stay buckled until the vehicle comes to a complete stop.
• Staff will determine the bus routes. No personal route requests allowed.
• Do not ask drivers to run errands or do special favors while being transported to/from the Oasis program.
• Move over to the window seat when entering the bus.
• Please do not wear perfume/cologne when riding the bus, as many are allergic to those scents.

Additional Expectations:

• New applicants to Oasis who live in assisted living and memory care communities are not appropriate candidates for Oasis participation.
• Participants who begin their Oasis participation while still independent, but then display changes in their physical and/or mental status, will be re-evaluated for continued eligibility in the Oasis program.
• Oasis and the Levine Jewish Community Center do not tolerate any type of prejudice.
• Arrive to lectures and programs on time, and do not leave until the speaker concludes.
• Turn off cell phones and do not speak to others during classes, discussions, and lectures.
• Do not enter rooms once programming has begun.
• Welcome new members and visitors, include them in activities, and wear your name tag.
• Please remember this is a Kosher facility. If you bring food into the building it must meet LJCC Kosher standards(Kosher guidelines available upon request).
• Support Oasis by keeping the facilities tidy by disposing of your used items in provided receptacles.
• Do not leave or store personal possessions at Oasis. Oasis will not be held responsible for any lost or stolen items.

Waiting List Policy

The majority of our classes and programs have open-ended enrollment. However, on occasion we will have to limit the number of participants due to the capacity allowed in a room or vehicle. At that time a “waiting list” will be created, and if a participant cancels his/her enrollment, waiting-list participants will be notified on a first-come basis.

Participant Dismissal Policy

ALL PARTICIPANTS UTILIZING OASIS ARE REQUIRED TO ABIDE BY OASIS' RULES AND CODE OF ETHICS. The Oasis Director or staff will use the following guidelines when addressing Participation Violations:

1. First Offense—Meet with participant to discuss the violation that occurred. Make a record of the incident. Advise the participant of his/her inappropriate behavior and that it may result in suspension from Oasis.
2. Second Offense—Meet with the participant to discuss violation. Add this incident to participant’s record. At the Director’s discretion participant could be suspended from transportation services and participation in programs and activities for one week.
3. Third Offense—Meet with participant. Give written notice to participant informing him/her of a temporary or permanent suspension from all services, programs, or activities. Participant must make an appointment with the Director to be interviewed prior to being allowed to re-enter the Oasis program.

Note:
The Oasis Director has the authority to override any and all Dismissal Policy procedures depending on the severity of the offense.

Incidents of Imminent Danger
Theft, violence, or any actions that can cause imminent danger to persons or property in our facility will immediately be turned over to Shalom Park Security. They will follow facility guidelines.

I agree to adhere to the criteria listed above.
